ALL Adult and Youth Authorizations are no longer accepted on paper forms.
Paper forms can only be used with prior communication with the Authorization Office.
Please use the Æthelmearc Paper Authorization Request form.
Cards may be expected within 3-6 weeks of receipt of completed paperwork.
